If you do not see your plan listed above please contact our office or refer to your vision benefit provider list.  Vision plans that we accept may not be listed because of the large number of name variations and affiliations.

Coverage will vary based on your specific plan.  Please contact your plan for information or we will be more than happy to verify coverage for you before your eye examination.  If you prefer, we will also help you with out-of-network paperwork.
